Problems with my voodoo5 AGP

3 days ago I purchased a voodoo5 5500 AGP. When I opened the box I found that my card did NOT come with the v5 quick install cd =(. What I did next is go to 3dfx.com and download the v5 drivers version 1.04. I installed these and it worked as far as I could tell. The problem begain when I installed Unreal Tournament. When I tried to get into the game the screen went crazy into a bunch of white and black lines and the computer did not recover. After 1 and a half days of trouble shooting I found out that my v5 was sharing an interrupt with my ethernet card. I removed the ethernet card and it WORKED!
Here is my current problem. After the card started working I upgraded my computer from win98 1st edition to win98 2nd edition. My v5 card no longer works and the blinking screen with black and white lines is back. I have installed multiple games and I always get the same black and white blinking screen. I tried reinstalling the drivers also. My computer is a Dell, 400 mghz pentium 2, v5 AGP, win98 2nd edition, if u need any other info just ask.
