Game Designer is a pretty general or wide term. i dont think anyone or just a couple maybe could call themselve that. I am going to school to learn about it, a BS in Game Art & Design. but i just started.

Well i have made map's for Q3, EF and mods for Armada and EF, and i also modded alot of the old sonic games for the Genesis, but i dunno about "Designer". I think that would be creating a new game from scrach. I dont think there's too meny people that make much outside of the freeware game of Chess that you can download, that will make you a game for free (assuming thats why you asked).
