I think perhaps a more simple, and far less creative theory would be that it wasn't any of the 'official' Sith Lords at all, but a Sith Lord who's existence had been a secret a whole time during the Hyperspace War and thus was never included in offical SW canon. Although it may well turn out to be one of the documented Sith Lords and Lucas Arts have simply decided to retcon (retroactive continuity) the story of the individual for convenience.

And Fyurii, although the title of Emperor is a male title with a female counter term, like 'actor' and 'actress' it may be that some are viewing them as interchangeable for the female gender and so just use the term generally. After all, even though the title of Pharaoh in the history of Ancient Egypt, there was at least one female Pharaoh that I know of named Hatshepsut. Besides, although in SW lore women are capable of as being just as powerful as many of their male counterparts, males are typically more aggressive, and even amongst Sith women would likely still be less so, and thus less likely to hold an official position of power, especially a position so demanding of total control of your underlings as the title of Emperor would require.

And that would probably explaing why there typically at least a few women on the Jedi Council (positions that would require logic and a peaceful disposition), and less prominently placed female Sith (an ideology that demands extremely turbulent emotional states from it's adherents).

Why hello I'd like to add to this discussion!!
Now I'm just throwing out an idea here: I think the mysterious sith Emperor is actually REVEN!!!! Think about it. He first attempted to conquer the galaxy in hopes that he could actually save it from this very threat. And after he was redeemed he realized he had to find some other meens to fighting the true sith.
Now remember that the who ever is the strongest sith is the one who rules them. Perhaps the only way to defeat the true sith was actually to join them, control them and eventualy lead them to there own desrtuction. After all they say The "Mysterious Emperor" lead his forces to victory, not by overwheliming odds, but by a brilliant, tatical plan (ring any bells). Also the Emperor, if you think about it, spared billions of lives by forceing the senate to sign the treaty. for even though the sith intend on dominating the whole galaxy, they have not yet crushed the Jedi as I believe they could have (Since the Jedi order had to basically bebuild itself after the "Jedi purge") I think The Emperor's useing this time to perfect his mysterious plan, and fullfil his greatest purpose: Defeating the ultimate threat (himself)!!! He was the only one who embraced the darkness and walked away from it. that's why he had to go alone. No one could do what he was born to do!
And before you throw this idea away because it does not line up with the timeline or dates, remember what Kreia said, "Nothing is impossible with the force". I'll bet with a little imagination, a little creativity, and a little magic that only Star Wars can deliver, this idea can actually be what the writers had in mind all along! Think about it

Two things.
1: There is no Reven, only Revan.
2: It's three hundred years later, and he wasn't alive a millenium before the first KoTOR.

It wasn't a brilliant tactical plan, but a desperate last ditch effort that brought the Sith into the position they are for TOR.
Go to the site, look at all the information available.
No brilliant tactical plan.
An Empire that's been existing longer than Revan, and lead by a single Emperor.

The only way Revan could be alive after all those three hundred years, is if the Sith Emperor was keeping him alive.
Then, only so he could see with his own eyes, how badly he failed the Republic.

Well, well, well, well, well. It's about time.

As Marka Ragnos said "The chains of the Grave cannot hold a Dark Lord of the Sith."

I for one do not care who the Sith Emperor is...I just want to kick some Jedi ASS.

Ummm. Just throwing this out there. In the trailer, one of the Sith has a mask that looks alot like Revan's. Just has more decorations and is Silver. Just pointing that out. In fact, all of them do except the one who, to me, resembles a bad a version of Malak.
