I think that is covered in the moding tutorial stickied to the top of this forum.

Planets can be made by copying an existing one in planets.xml and changing what you need. (You link the ground map to a planet, not the planet to the map.)
Because I've never created any campaigns, I'm not sure about it. You have to edit campaigns_singleplayer.xml and one of the sandbox files, but I have no experience in it (and because I'm busy at the moment I don't have time to figure out in for you). Try asking somebody who' done this.
Making the map: Yes, it's possible.
Making the planet and the campaign: No, you must code it.

Because modding has become a competition, not fun, I don't want to take place in the public modding anymore.

If you're giving a planet a custom map that you made look for these 2 lines--

<Land_Tactical_Map> _Land_Planet_AbregadoRae_02.ted </Land_Tactical_Map>
<Space_Tactical_Map>_Space_Planet_AbregadoRae_01.t ed</Space_Tactical_Map>

Of course you'll have to insert your own map names. Hopes this helps you.

Well, if you were adding a new planet you would almost certainly need a new map. You create the map with the editor, then insert the name of the map into the code string that deals with the ground map.

Edit: What he ^ said

When you put in a new planet it should show up on the in-game map automaticly........... At least, that worked for me.

Are you sure? I think it must be added to the campaign's xml file before it shows up in the game.

Yeah in order to get the planet in the campaign. I thought he was talking about the map of planets that you see while playing GC.
