Oh Noes!
 
My battery packs, keep disconnectin’ all in the middle of a game, there fully charged! I've noticed when I shake my controller my batteries packs become lose and disconnect my controller. So what’s the issue the Battery pack or Controller?

I'd say its the battery packs.

The clip holding it on might be breaking, causing it to fall out.

I only have wired controllers though so I am just making a guess.
