in cs:s de_dust 2 stand near the double doors, throw a grenade but throw just on the narrow part of the door instead of the opening of teh door, grenades returns right in you face and BANG!

I was hiding behind someone in Battlefield 2 once (reloading), someone shot them in the face, for some reason, it went through their head and killed me. They lived and I didn't.

God, that happens to me all the time. I hate when that happens.

I remember this part in Jedi Outcast in the last level I think it is, where there's these three paths in a room. Two are literally dead ends.

One of them looks like just a simple dead end, but you die if you go too far.

The other, which scared the shit out of me the first time, looks like a mirror. You see yourself (Kyle) in the reflection. If you stay there to look, Desann jumps out from behind you and kills you.
