I have the 5 soul gems needed to bring to this witch, but 3 of them have some sort of charge in them. When i try to charge one of my items with the game it disappears after use. What am I doing wrong?
There is a readme file in the .rar, if you read that you should know everything. All I had to do was reading it and I wasn't a vampire anymore
But to answer your question directly as to who it is who sells the potions, it's the Dark Elf female in the Skingrad Alchemy shop (the one that asks for the bill of "doing it" with dead bodies).
I've checked the ReadMe file, only thing that is written there is where to go to get the potion, so if you would please tell where i should extract the file, then it would be very nice of you thanks..

You have to find 5 empty grand soul-gems. Those charged with a soul won't help you. Maybe there is a way to get rid of the charge at the Arcane University but I didn't try that yet.
- Empty Grand soul gems = Check the mages guilds
- Onions = Check bartenders.
- Bloodgrass = Check alchemist
- Nightshade = -Check the area around the witch's house
- Check Alchemist or Mages guilds
Let's all face a fact.
The cure vampire thing is a real pain in the ass.
First off you have to locate this bitch living in the forest.
Travelling for about 15 minutes you think thats it now i can get rid of this shit.
But hell no.
Now you have to find these 5 gems.
Searching all around the tombs and ruins gathering 5 damn gems, or going to the mage shop, whatever.
Then after searching for this shit about 2 hours you think, finally this shit is behind me.
Hell damn no.
Now you have to go look for the hardest pain in the ass shit things you could ever dream about.
Some plant from oblivion, stab som local, grochery shopping, flowerpicking in the forest and messing with the baddest fucking vampire ever.
What the fuck! All i did was killing a few vampires for the emperor and i get this shit?
Thanks to the earlier walkthrough and the and game's strategy guide, I finally completed the Cure Vampirism quest! It definitely took up a sizeable chunk of time. I'm not so ticked that the cure quest was so tedious, but I am pretty ticked that it wasn't more apparent that you'd have to go through all this to get rid of vampirism. I mean, I didn't even know I had gotten it before it was too late (I saw a message about a bad dream, but I didn't know what it meant). At any rate, for those who CHOOSE to be a vampire, I can understand that they would have to go through this quest in order to get rid of it if they so choose. However, there should be a very clear message saying that if you don't cure yourself within 3 days of contracting it, it will be pretty-much permanent unless you go through this quest.
That said, I have a question:
Now that I have taken the cure, what happens if I get it again? The lady that made the cure said she could make more, so is it as simple as going back to her if you get it again and she'll make more, or do you have to go through the whole soul gem and ingredient hunt for her to make more if you get it again (which could be harder the second time around)? Hopefully, now that she has made the cure, you could get more if you get it again!

Hm, that's strange.
Extract the file to:
C:\Program Files\Bethesda Softworks\Oblivion\Data
Then launch the game and choose Data Files, from there check VampireCurePotions 1.0.esp (Player File).
sneak into the ladies house and in the fireplace that is not lit there is a trap door. Inside is alot of goodies including alot of soul gems. I hope this helps you vampires out
