Spawned Null Entity

If anyone could help with this problem I'd appreciate it. Whenever I die and load again, or load another saved game, the game quits with the Error "Spawned Null Entity". Ive tried re-installing and it doesnt work. It's really not that fun playing with this problem so I'd like it if someone has advice.
